4. Orthodontic Differential Diagnosis
We will determine extraction or not-extraction treatment through the orthodontic diagnosis.
When I decide whether to choose extraction or non-extraction treatment, I consider the aesthetics,
combination factor, and securing the space in order.
The aesthetics are evaluated through the appearance, and the combination factor is evaluated by
cephalometric analysis.
However, whether or not to secure space is considered in various aspects.
Whether teeth can be up-righted or mesially angulated, mesially or parallel rotated, expanded or
enlarged, and also adjustment of the occlusal plane should be evaluated. It is possible to be evaluated
through the extraction index, which is calculated by using the angulation of the anterior teeth.
In the case of premolar extraction treatment, the PEG index can provide a simple guideline, whether to
extract maxillary and/or mandibular premolars and whether to extract the first or second premolars.
It also explains the diagnosis of Class II and III malocclusions, and presents a problem list and solutions.
